SMP Health – St. Raphael is seeking compassionate and experienced Registered Nurses to provide quality nursing care in a Skilled Nursing Facility environment. The successful candidates will deliver patient-centered care, monitor residents’ health conditions, administer treatments, and support the overall wellness of geriatric patients while maintaining professional nursing standards.

Registered Nurse

Qualifications:

• Licensed Nurse with valid NCLEX qualification.
• Minimum of 2 years of bedside nursing experience.
• Must be currently practicing in a hospital setting.
• Willing to work in a Skilled Nursing Facility (SNF) environment.
• Willing to provide care for geriatric patients.
• Strong clinical assessment and patient care skills.
• Excellent communication and interpersonal skills.
• Compassionate, reliable, and patient-focused.
• Ability to work independently and effectively as part of a healthcare team.
• Physically and mentally fit to perform nursing duties.

Responsibilities:

• Provide comprehensive nursing care to residents in a Skilled Nursing Facility.
• Assess, monitor, and document residents’ health conditions, vital signs, and progress.
• Administer medications, treatments, and nursing interventions according to physicians’ orders and care plans.
• Develop, implement, and evaluate individualized nursing care plans.
• Provide compassionate care and support to elderly and geriatric patients.
• Monitor changes in residents’ conditions and report concerns promptly to physicians and healthcare teams.
• Collaborate with healthcare professionals, caregivers, and families to ensure continuity of care.
• Maintain accurate and complete nursing documentation and medical records.
• Educate residents and families regarding health conditions, treatment plans, and care instructions.
• Follow infection control procedures, patient safety standards, and facility policies.
• Perform other nursing-related duties as assigned by the employer.

Requirements:

• Must have a valid passport for more than 3 years
• Employment certificates
• NBI clearance valid for at least 1 year with “no derogatory / no record on file” remarks
• PEOS certificate
