HOME CARE NURSE
Qualifications:
- A minimum of 2 years of relevant nursing experience is required.
- Candidates must be a Professional License (Passed Board/Professional License Examination) holder.
- Female applicants are preferred for this role.
- Strong patient assessment and clinical evaluation skills.
- Experience in providing direct patient care in home care, hospital, clinic, or healthcare settings.
- Knowledge of dialysis patient care and the ability to evaluate patients’ responses to dialysis treatments and medications is an advantage.
- Ability to develop and implement individualized patient care plans.
- Excellent observation, monitoring, and documentation skills.
- Strong verbal and written communication skills.
- Compassionate, patient-centered, and professional attitude.
- Ability to work independently and manage patient care responsibilities effectively.
- Good interpersonal skills and the ability to communicate patient concerns accurately to physicians and healthcare teams.
Responsibilities:
- Provide direct nursing care to patients in their homes according to prescribed treatment plans and physician instructions.
- Assess patients’ physical, emotional, and medical conditions and monitor changes in their health status.
- Develop, implement, and update individualized patient care plans.
- Administer medications, treatments, and nursing procedures as prescribed.
- Monitor patients undergoing dialysis treatment and evaluate their reactions to treatment and medications when applicable.
- Record vital signs, symptoms, and patient progress accurately.
- Identify health concerns and promptly communicate important findings to physicians and healthcare professionals.
- Educate patients and family members regarding medications, treatments, and proper home care practices.
- Provide emotional support and promote patient comfort, dignity, and well-being.
- Maintain accurate nursing records, reports, and documentation.
- Follow infection control, safety, and healthcare protocols at all times.
- Respond appropriately to patient emergencies and coordinate necessary medical assistance.
- Listen carefully to patients’ concerns and communicate their needs accurately to the attending physician and healthcare team.
- Ensure the delivery of safe, compassionate, and high-quality home healthcare services.
