HOME CARE NURSE

Qualifications:

• A minimum of 2 years of relevant nursing experience is required.
• Candidates must be a Professional License (Passed Board/Professional License Examination) holder.
• Female applicants are preferred for this role.
• Strong patient assessment and clinical evaluation skills.
• Experience in providing direct patient care in home care, hospital, clinic, or healthcare settings.
• Knowledge of dialysis patient care and the ability to evaluate patients’ responses to dialysis treatments and medications is an advantage.
• Ability to develop and implement individualized patient care plans.
• Excellent observation, monitoring, and documentation skills.
• Strong verbal and written communication skills.
• Compassionate, patient-centered, and professional attitude.
• Ability to work independently and manage patient care responsibilities effectively.
• Good interpersonal skills and the ability to communicate patient concerns accurately to physicians and healthcare teams.

Responsibilities:

• Provide direct nursing care to patients in their homes according to prescribed treatment plans and physician instructions.
• Assess patients’ physical, emotional, and medical conditions and monitor changes in their health status.
• Develop, implement, and update individualized patient care plans.
• Administer medications, treatments, and nursing procedures as prescribed.
• Monitor patients undergoing dialysis treatment and evaluate their reactions to treatment and medications when applicable.
• Record vital signs, symptoms, and patient progress accurately.
• Identify health concerns and promptly communicate important findings to physicians and healthcare professionals.
• Educate patients and family members regarding medications, treatments, and proper home care practices.
• Provide emotional support and promote patient comfort, dignity, and well-being.
• Maintain accurate nursing records, reports, and documentation.
• Follow infection control, safety, and healthcare protocols at all times.
• Respond appropriately to patient emergencies and coordinate necessary medical assistance.
• Listen carefully to patients’ concerns and communicate their needs accurately to the attending physician and healthcare team.
• Ensure the delivery of safe, compassionate, and high-quality home healthcare services.
