Community Worker

Qualifications:

Responsibilities:

  • Assist community members in accessing social, health, welfare, housing, and other support services.
  • Assess clients’ basic needs and provide appropriate information, guidance, and referrals.
  • Support individuals and families experiencing social, financial, housing, or other personal challenges.
  • Organize and participate in community programs, activities, workshops, and events.
  • Provide practical assistance and emotional support to clients when required.
  • Conduct home visits or meet clients in community settings to provide support and follow-up.
  • Maintain communication and coordinate with government agencies, healthcare providers, charities, and other service organizations.
  • Advocate for clients’ needs and help them understand available services, programs, and community resources.
  • Assist clients in identifying suitable support options and accessing relevant services.
  • Maintain accurate and confidential client records, case notes, referral details, and reports.
  • Follow workplace policies, confidentiality requirements, and professional standards when working with clients and families.
