Requirement to Register in the One Health Pass Platform

(All people going to the Philippines — OFW, non-OFW, foreigners — are required to enter the One Health Pass to speed up the process at the airport.)

So far, only diplomats, foreigners engaged in medical services, foreign spouses and children of Filipino citizens traveling with them, and others with humanitarian cases are allowed to enter the Philippines amid the pandemic.

Travelers will fill out the corresponding form three days before travelling to the Philippines to generate a transaction number, according to the BOQ.

A day prior to boarding the flight to the Philippines, the passenger will be required to fill out an electronic health declaration checklist and will be given a QR code

To get the QR code, you will have to accomplish the One Health Pass Declaration Form (e-HDC) which will require you to submit personal information, health status and travel history. The forms should be filled out individually.

All travelers to the Philippines will need to register in the “One Health Pass” platform.

This a 2-STEP REGISTRATION PROCESS:

Step 1 – to be accomplished at any time on or before your departure date: Register through https://www.onehealthpass.com.ph/e-HDC and completely fill out all required fields. Upon filling out the form, please ensure to save a copy of your unique transaction number, which you will need when you are at the airport to accomplish Step 2 below to complete your e-HDC registration.

Step 2: to be accomplished at the airport, prior to boarding your flight to the Philippines: Before boarding your flight, answer the e-HDC at https://www.onehealthpass.com.ph/e-HDC by entering your unique transaction number to retrieve your record. Upon completing all the information, you will receive your unique QR code which you need to present before boarding and upon arrival.

All information shared in the One Health Pass is collected and processed solely by the Bureau of Quarantine.

All Philippine-bound travelers are required to register before the date of travel to the Philippines.

For Seafarers

All Seafarers will no longer undergo RT-PCR test upon arrival at the airport, instead will be directly billeted to the approved OWWA quarantine facility/hotel. Here is the updated guidelines for arriving seafarers following IATF 114; 116 and 124-b:

STEP 1 PRIOR FLIGHT SCHEDULE:

A. Three (3) days prior their flight/departure, seafarers must register at https://onehealthpass.com.ph/e-HDC/ AND https://ecif.firstaide.ph/pre#/ and secure a QR Code. This registration is for both air and sea crew arrival.
B. Passengers arriving in Cebu (register not earlier than 5 days before flight departure): https://mactancebuairport.com/covid-19-registration

D. Download and install the Traze Contact Tracing App (https://www.traze.ph/download.php).

STEP 2 UPON ARRIVAL AT THE AIRPORT
A. On the day of disembarkation, seafarers shall proceed to the One-Stop Shop (OSS) facility and/or Firs aide Booth for briefing and verification of documents (provide copy of your SRB). All seafarers must be briefed properly on the latest safety and health procedures of the national government, as well as their rights and responsibilities throughout the process. Seafarers must also accomplish the OWWA Project Care Form.
B. Proceed to the Bureau of Immigration for clearance prior to retrieval of luggage(s).
C. Proceed to MARINA Help Desk.
D. Transport is arranged by Magsaysay/OWWA for transfer from airport to accredited OWWA quarantine facility

Note: For fully vaccinated crew: present your vaccination card to the dedicated Bureau of Quarantine (BOQ) representative/ One-Stop-Shop (OSS) for verification.

STEP 3 WHILE ON QUARANTINE
A. The seafarers will be billeted to OWWA accredited quarantine facility for the first 10 days.
B. Mandatory RT-PCR test at the hotel/quarantine facility will be conducted on the 7th day from the date of arrival. (e.g. If crew arrived in the Philippines 07 May 2021, he or she must immediately proceed to his/her designated quarantine facility on the same date while his/her COVID-19 test must be conducted on 13 May inside the facility.)
C Seafarer shall observe all quarantine procedure while waiting for results and/or completing the 10 day quarantine period

For fully vaccinated crew

  • Seven (7) —day facility-based quarantine upon arrival is required. The BOQ will strictly monitor symptoms while in the facility quarantine for seven (7) days.
  • No RT-PCR test is required unless crew manifest symptoms while at quarantine facility.
  • Upon completion of the seven (7)-day facility-based quarantine, the BOQ shall issue a Quarantine Certificate indicating therein the vaccination status

STEP 4 UPON RELEASE OF RT-PCR TEST RESULT
A. Upon receipt of negative RT-PCR test results, the Bureau of Quarantine will issue a Health Certificate indicating the actual number of days on quarantine.
B. Seafarers will need to present the RTPCR negative result and BOQ Health certificate to check out of the quarantine facility.

  • For Luzon-bound seafarers, proceed to PITX in Paranaque
  • For Visayas and Mindanao-bound including Palawan, proceed to NAIA PAL Terminal 2 for processing. Fill-out the necessary forms and wait for your boarding announcement.
  • Upon arrival at the airport of destination, seafarers will have to report to RTF/PTF/OWWA Station or LGU for accounting and instructions.
  • RTF/PTF OR LGU WILL PROVIDE FREE TRANSPORTATION TO OF FINAL DESTINATION.

STEP 5 COMPLETION OF 14 DAY QUARANTINE
A. Seafarers will complete the remainder of the M-day quarantine (total of 14days) at the assigned city/municipal/barangay quarantine facility, or at home if permitted by your LGU.
B. Once released from quarantine by LGU, seafarers are advised to continue observing minimum health standards, such as wearing of face mask, maintaining physical distance and washing of hands. Also, always keep your immune system high by having enough sleep, eating the nutritious, and doing regular physical exercise.
C. LGU has the discretion to implement additional measures to fully vaccinated crew e.g. RT-PCR Test or any COVID-19 Test may be required.
