• NBI Clearance
• Police Clearance/Barangay Certificate
• 2×2 ID Picture
• MDF Pag-ibig Form
• Diploma or TOR/F137
• PSA Birth Certificate
• PSA Marriage Certificate (only if Married)
• Pre-medical Exam (must be submitted after orientation)
• VaxCert/Yellow Card
