HOME CARE NURSE
Qualifications:
- A minimum of 3 years of relevant working experience is required.
- Candidates must hold a Professional License / Passed Board or Professional License Examination.
- PRC License is required.
- Preferably a HAAD passer with DataFlow verification.
- Minimum of 3 years of nursing work experience.
- Strong clinical assessment and patient-care skills.
- Knowledge of medication administration, IV therapy, vital signs monitoring, and nursing procedures.
- Good communication and interpersonal skills.
- Able to work independently in a home healthcare environment.
- Able to follow physician orders and established nursing care procedures.
- Compassionate, patient, responsible, and professional.
- Strong attention to detail and accurate documentation skills.
- Physically and professionally capable of performing home healthcare nursing duties.
Responsibilities:
- Provide direct and compassionate nursing care to patients in a home healthcare environment.
- Conduct physical examinations and assess patients’ overall health condition.
- Interview patients and collect detailed medical and health histories.
- Monitor and record vital signs, symptoms, and changes in patients’ conditions.
- Develop, implement, and evaluate individualized nursing care plans in coordination with physicians.
- Administer prescribed medications, injections, IV therapy, and other treatments according to physician orders and applicable nursing procedures.
- Monitor patients for adverse reactions to medications, treatments, and procedures.
- Manage IV lines and monitor patients receiving intravenous therapy.
- Carry out physician orders accurately and promptly.
- Perform or assist with diagnostic tests and procedures as required.
- Operate and monitor appropriate medical equipment used in patient care.
- Observe patients closely and promptly report changes in health condition to physicians or supervisors.
- Provide wound care, dressing changes, and other nursing interventions as required.
- Assist patients with activities of daily living and other care needs when necessary.
- Educate patients and family members regarding medications, treatment plans, hygiene, disease management, and follow-up care.
- Provide emotional support and reassurance to patients and their families.
- Maintain complete, accurate, and up-to-date patient medical records and nursing documentation.
- Follow infection prevention and control procedures and patient safety standards.
- Maintain proper handling, storage, and documentation of medications and medical supplies.
- Coordinate with physicians, caregivers, therapists, and other healthcare professionals to ensure continuity of care.
- Respond appropriately to emergencies and follow established emergency procedures.
- Maintain patient confidentiality, dignity, and privacy at all times.
- Perform other nursing and home healthcare duties as assigned by management.
