HOME CARE NURSE

Qualifications:

• A minimum of 3 years of relevant working experience is required.
• Candidates must hold a Professional License / Passed Board or Professional License Examination.
• PRC License is required.
• Preferably a HAAD passer with DataFlow verification.
• Minimum of 3 years of nursing work experience.
• Strong clinical assessment and patient-care skills.
• Knowledge of medication administration, IV therapy, vital signs monitoring, and nursing procedures.
• Good communication and interpersonal skills.
• Able to work independently in a home healthcare environment.
• Able to follow physician orders and established nursing care procedures.
• Compassionate, patient, responsible, and professional.
• Strong attention to detail and accurate documentation skills.
• Physically and professionally capable of performing home healthcare nursing duties.

Responsibilities:

• Provide direct and compassionate nursing care to patients in a home healthcare environment.
• Conduct physical examinations and assess patients’ overall health condition.
• Interview patients and collect detailed medical and health histories.
• Monitor and record vital signs, symptoms, and changes in patients’ conditions.
• Develop, implement, and evaluate individualized nursing care plans in coordination with physicians.
• Administer prescribed medications, injections, IV therapy, and other treatments according to physician orders and applicable nursing procedures.
• Monitor patients for adverse reactions to medications, treatments, and procedures.
• Manage IV lines and monitor patients receiving intravenous therapy.
• Carry out physician orders accurately and promptly.
• Perform or assist with diagnostic tests and procedures as required.
• Operate and monitor appropriate medical equipment used in patient care.
• Observe patients closely and promptly report changes in health condition to physicians or supervisors.
• Provide wound care, dressing changes, and other nursing interventions as required.
• Assist patients with activities of daily living and other care needs when necessary.
• Educate patients and family members regarding medications, treatment plans, hygiene, disease management, and follow-up care.
• Provide emotional support and reassurance to patients and their families.
• Maintain complete, accurate, and up-to-date patient medical records and nursing documentation.
• Follow infection prevention and control procedures and patient safety standards.
• Maintain proper handling, storage, and documentation of medications and medical supplies.
• Coordinate with physicians, caregivers, therapists, and other healthcare professionals to ensure continuity of care.
• Respond appropriately to emergencies and follow established emergency procedures.
• Maintain patient confidentiality, dignity, and privacy at all times.
• Perform other nursing and home healthcare duties as assigned by management.
